Please fill out the form completely, listing all of your items of value.

• If you wish to have additional liability coverage applied to your move, you must list all items of high value to include furniture that are valued over $1000 as well as items we will be packing that are valued over $300. This information is required so that we can properly insure your items with our carrier as well as let the crew know which items need to be handled with extra care and precautions.
• If you do not wish to have additional coverage applied but would like to point out items of interest whether it be valued items or sentimental items, you may do so by listing the items within our valued items list. If you do not wish to have additional coverage, leave the “declared value amount” at zero.
• NOTE: All items will be wrapped and protected regardless of being listed however in order to apply additional coverage this form must be filled out per our insurance provider. As for those not seeking additional coverage, this allows the crew to understand what items you wish to have wrapped & protected above and beyond.
